Mr Musk may use the phrase, but he nicked it from someone else (no surprise there, then...) The first recorded use I've found was by NASA engineers in 1967 referring to a Saturn 503 S-IVB test at the Douglas facility in Sacramento that did not go as planned. By the 1970s the expression was in widespread use in the aviation industry.
